This is an opportunity for you to let us know what is working, what isn't, and how you think the DSASC could serve you and your family better, so please, answer in as much detail as you are able.

This survey is anonymous, and questions without a * are completely optional.
Completed surveys will be aggregated and the information will be shared with the DSASC executive committee to help make decisions to help best meet the needs of our members. Some aggregated information (not long-form written answers) may also be shared with the membership at an AGM or in a newsletter.
